Be as clear and concise as possible while still being polite. For example: Please cancel my subscription to Catnip-R-Us magazine. My account number is 531meow. (or list your full address if you do not have an account number).

First check the cancellation clause if you have a written contract and make sure your terms match theirs. Let them know that you are cancelling, but remember you don't have to give a reason (unless stated in your contract). Send an email if possible so as to give the maximum amount of notice.
