Most PS2 mods, be they for simply playing imported games or for playing illegally copied discs, require a chip to be installed inside the machine. The exact steps vary from mod chip to mod chip, but generally you have to open the machine and solder the chip into the system's motherboard in a specific way.

Hardware mods require you to actually open up the PS2. The solder or clip on a new chip and load new operating/boot software to the PS2.

Mod is short for "modifying". So really it could be caning the console in any way. Typically though it means installing a mod chip. Which is a chips that modifies ta console to to stuff it wasn't originally designed to do.
