How do you install a grab bar onto ceramic tile without breaking the tile?

Use a diamond or carbide drill bit to drill holes. Use masking tape to make a cross on the area you want to drill to prevent the tile from breaking apart. Spray water constantly while drilling to reduce heat and friction. And drill slowly with constant breaks.

Use a specially designed drill bit. These bits are covered at the tip with actual slivers of diamond shards. Use the bits to drill holes and insert molly bolts into the holes. You may now install the bars with out cracking the tile. Once the secrews are in place use caulking around the base to seal out water.

You can cement the grab bar ends to the ceramic tile or you can use a drill bit, specific for masonry, to drill holes in the tile without breaking it.

You can use a low impact Drill to pre drill the holes for the screws. You can also use duct tape over the area first to prevent some cracking. You can also install it in to the grout line.
