The Vet will look at the dog's ears and he usually will prescribe some drops that you can put into the ear canal and massage in then let him shake.

Take your dog to the vet and they'll give you drops that will kill the mites. Then regular cleaning of the ears.

Ear mites can be removed by using mineral oil and a q-tip. Also there are some shampoos that a vet can prescribe that will help, prescribed medication too.

Ear mites need to be treated with medication from your vet. Co-ops carry the medicaion, however do you know for sure that it is ear mites? Dirt and debris in dogs ears can cause the same behavior. The vet can swab the ears and look under the micoscope to confirm whether it is or isn't.
