You can file on line or call a claim center. There's 3 requirements: Employer is subject to Unemployment Insurance laws, your gross income was $2,500 earned over 2 calendar quarters of a base period, and seek full time employment. They need your name, address, social security number, name address, telephone of last employer, last day worked and reason for leaving.
