Lots of stores sell these costumes, but if you want to create your own, you can. You just need a white dress and create from a piece of paper a red cross. You can ask a hospital if they have a little paper hat you can wear with it.

Use white clothes that you already have, and embellish with nursing accessories. These could include steathescope, thermomitor, bloodpressure cuff, and cardboard nurses hat.
