To change your surname go the county courhouse and file a position for name change. A public notice of your intent will need to be posted for a short time. This usually can be posted in your local newspaper. If there is no objection, you name change should be processed.

You go to court and request a name change document, and it might cost you a little bit of money. If you are getting married then you just sign you name on the marriage license and the court will do the rest. The same is true of a divorce, there is just a box you can check.
