You must file through the Family Court after you complete all the forms required by the state and provide requested documentation. An attorney would be helpful.

Assuming the person is willingly passing you temporary guardianship to their child it's a simple court process in which you both visit the municipal and sign some documents.

You usually go to the Probate Court in your County and make an application. There are fees associated with these filings, you may be able to qualify for a waiver.

You need to talk to a social worker first. Or whoever is involved in the case. You can obtain the papers to fill out from an attorney.

Go to your nearest courthouse and ask the clerk there for a form to obtain guardianship and inform them if this is for a minor/adult and fill it out completely.

You can consult a lawyer and have temporary guardianship papers drawn up. Or, if you and the child's parent agree to the temporary guardianship, you both can write a statement detailing what this informal agreement includes. Is it a medical and educational guardianship? What will be the duration of the guardianship? These items should be included in the notarized paperwork.
