Check wires (probably from tank) to source, connections. Try new filter, relay, if cheap. At a good parts store they'll know what usually gets replaced on your car and have repair books. In tank pumps use fuel as coolant and lubrication. If yours fails at low fuel levels, it's probably time to replace, meantimes keep tank half full or more.

It depends on if your fuel pump is in the gas tank or on the engine? if it's in the fuel tank check the fuse in the fuse box. You may have water in your gas tank.

You should take your car in to a repair shop to find out. Or go to an auto parts store and get a manual for the car, tell them the make and model of the car.

Well intermittent failure is a possible sign of a dying pump. Usually there is no way to rebuild or repair a factory pump so you will have to install a new one.

Check and see if the fuel line needs replacing. Usually this is an easy fix. Sometimes a simple replacement line can free up the system.
