First of all it is illegal to create and sign a doctors note. Doing this can lead to prosecution under several different laws and generally is not even remotely worth the risk. If you still feel you must do this first acquire a doctors note and then use a scanner.

Just write the note and scribble a lot like a true doctor would. Then practice a doctor's name a few time son a separate sheet of paper. Then scribble the doctors name on the note. But be careful because they may call and check the note.

If you had any doctor's note before, try to erase the date and put the new day on it. Try to make it look like real or you will be in trouble

Don't even try it. Forging a doctors signature can get you in HUGE legal trouble at the worst. If you don't get charged with impersonating the doctor you could also get in huge trouble with your parents and your school. You could very well be expelled for your inappropriate behavior.
