I guess you would tell a family member or have it written up in your will or in your power of attorney.

You make the request of the attending physcian. Anyone can ask for an autopsy and there is usually no charge.

Nearly all hospitals can provide an autopsy for patients who die there. Otherwise, search for "pathologists" in your area and contact them to see if they can help.
