Yes, I feel you do need to wear a mask when cleaning concrete. My reasons for this is that some people clean concrete with caustic substances and you wouldn't want them to get in your face or even breathe them in. Also if just power washing, the concrete is porous and can send debris up towards your face.

It's a good idea to wear a mask when working in any kind of environment with particulates in the air. That way your respiratory system is protected.
