The airlines has strict rules about kids flying unattended, including age restrictions. Contact the specific airlines for details as they vary from one carrier to another. The airline must be notified, the child boarded and seated where s/he can be monitored. The parent on the other end must be there when the kid arrives.

Usually an informal phone call is all that you need. Communicate first, then if a letter is needed proceed. talk to both the father and the mother. that way you have both bases covered.

Im pretty sure you do just so they don't think that the child is just running off on his or her own. But to make sure you should check the airport or if he or she is riding on a bus ask the bus station. Someone there will be able to tell you.

yes if it is out of state then yes you have to have an approvel it depends on the situation.or if it is court ordered or if they are traveling by them self then you have to have approovle

yes i need a approval for a child for travelling from his father to mother in case they are living alone an they have been divorced legally. This is the only case we need for an approval from the child when it travels. Because there are some problem for the both if the child faces any issues in between.
