A DD149 is used for a correction in your military records. Depends on what was corrected. See Army Career Counselor asap, must be done in specific time frame, check qualifications.

No, DD Form 149 is an Application for Correction of Military Record. You need to locate and fill out DD FORM 4/1, DD FORM 4/2 and DD FORM 4/3.

You can reenlist post dd form 149 if it results in a better discharge rating. If you were separated with anything that looks negative on your record and the dd form 149 made an improvement to that discharge it will help you reenlist into any service.
