A cake in shape of /with an image of champange glasses would be creative. That would also be a nice classy hint that he is allowed to drink now.

First think of his favorite things. Sports, Music or Group, pets etc. Then I would work on that theme. You might get him tickets for a sports or music event and put them on the top of the cake. Or make a cake like a sports car.

ive seen sometimes for graduation cakes where the cake places will take a picture of the persona nd put it on the cake, what if you made them a state id cake indicating drinking age?

Well a 21st birthday is a big thing since the person can drink legally so I would suggest a cake that reflects drinking and fun but in moderation since it is supposed to be the night of his first drink.
