Having 12 men on the field when the offense leaves its' huddle. It is illegal to have more than 11 players on the field.

12 men breaking the huddle means that 12 men are on the field at the start of a play, one more than the legal amoun of 11 players.

According to the NFHS rules, "12 men breaking down the huddle" means that only 12 men are allowed on the field at one time.

There is no rule stating a penalty but that the substituted player must leave the field immediately, which is usually 2 - 3 seconds.
