Severe constipation can cause back pain with a bowel movement. I would first try increasing the fiber in my diet and if this is not helpful over the counter laxatives. If the problem persists, see your doctor for further evaluation.

Your back may be hurting due to the strain of hardened waste during your bowel movements - this could be caused by lack of fiber in your diet which will prevent waste from easily passing through your colon.

Most likely you have been constipated or are currently constipated. When you are this can cause your muscles to strain when using the bathroom. If you have been straining for awhile then you will see that you have pain in many areas. It could also be that you pulled a muscle another way and using the bathroom irritates it.

When you strain to have a bowel movement, you place strain on the lower spine and the surrounding muscles and disks.
