It depends. What engine is in the van? 2.5L engines are known to have head gaskets leak and it could be coolant burning (white smoke). 3.0L and 3.3L can leak oil into the engine and burn through the exhaust (blue smoke). If it's cold out it could be normal steam.

When a head gasket is blown you will notice heavy white smoke coming from the tailpipe that has a distinctive anti-freeze smell to it. If there is no anti-freeze smell the tailpipe, check for oily residue from a vehicle that is burning oil and needs to have it's valves checked out.

Without knowing more about the car and it's service history, I can't say for sure. Smoking indicates that there is something that is burning off in the cylinders during combustion. If it is blueish smoke then you an oil leak. If it is white and sweet smelling then it is coolant from something like a leak or cracked head.

There isn't a single answer to this, it all depends on the color of the smoke and in what situation you see it coming out the tail pipe.
