What is the proper way to write an address on an envelope?

I am confused about what goes on each line.

The first line of the address is for the name; the second line is for the street address and apartment number if there is one; third line is for the name of city and state or province and the fourth line is for the country (if different from where you are sending) and the postal code.

The name of the addressee goes on the first line, second line is street address or PO Box, third line is City, State and Zip-code. Miscellaneous goes above street address.

The USPS requests: first line-name of individual or organization; next, organization, if needed; next: street number-street name w/ proper abbreviation AND TWO LETTER STATE ABBREVIATION followed by the zip code.
