Attach the engine block to a hoist to hold the engine, unbolt engine from the frame at the mounts, lift with the hoist and secure, replace the rubber mounts, reassemble.
Start by jacking up the engine a bit to take weight off the mounts. Remove the mount from the frame then the mount from the engine and change the mount.

Take weight off the mounts by supporting the engine with a jack. Then inspect the mounts for damage. Next remove the radiator, cross member, and anything else in the way.
