Soak the toy (squeeze the air out so it fills up) in hot water and bleach (use 1/2c per gallon of water) for 10 minutes. Shake toy to squeeze out solution. Keep filling/squeezing until the water comes out clear. Rinse with clean water to remove all bleach residue.

You can soak your childs bathtoys in steaming hot water with a small amount of bleach. Scrub them with a scrub brush to get the wate build up and soap scum off. If you can't get them clean then you should toss them.

It depends what they are made of, boiling them will sterilize them so that's probably best. You could also toss them into the dishwasher (not with dishes) or give them a wipe down with baby wipes.

You should soak them in a very light bleach solution. Use about a 5% to 95% water solution. Soak in warm water for about 15 minutes. You can do this about every two weeks if necessary.
