Referring to the person in question by different gender titles, treating someone as more masculine or feminine or talking to them as if they were the other gender.

Gender confusion tests involve testing the subject who is gender confused. It also involves testing a subject who others think is of a different gender than that person claims to be. Gender confused can mean someone who feels they were born as the wrong sex or someone who would prefer being a different sex.

The "tests" are mostly a clinical interview with a psychologist. Search for the DSM-IV classification for Gender Identity Disorder to get an idea of the symptoms they are looking for.
