Give a dated notarized agreement containing the name, signature, and address of the borrower and the name and address of the lender, along with the amount borrowed, the terms of prepayment and interest rate. If it is to be paid in installments, set forth the installment dates and amounts.

Name and address of payer and payee, amount of loan, interest rate, annual percentage rate, total cost of loan, terms and conditions of payments, penalty for late payments, etc.

It will have the names of all parties involved, the amount of money and the time frame when it has to be repaid adn the consequences if not.

When you write a promissory note be sure to include your name, the person's name you are making an agreement with, list what the agreement is, and if payment is to be made list how much and when the payment is to be made.
