What happens to the Adam's apple of a transgender male?

Except in the case of surgical modification, absolutely nothing changes. The Adam's apple's size isn't affected by the hormone treatments. Often, however, male-to-female transexuals will have plastic surgery done in order to reduce the visibility of their Adam's Apple, often alongside vocal chord scrapings or other surgeries to help alter the voice.

The "Adam's apple" is developed with normal growth, and cannot be reduced through hormones, etc. There is surgery that will reduce this cartilage protuberance. It is recommended that this is done in order for the client to feel fully socialized, but is not physically imperative, and should be secondary surgery.

Once the Adam's apple develops it is there. A transgender male will continue to have the Adam's apple. There is plastic surgery called cartilage shaving which can shave down the Adam's apple and make it smaller so it can't be seen.
