Often when someone makes this statement they are referring to someones lack of leisure. Usually someone would tell you this if you often seem uptight or busy.

Usually this phrase means to slow down, take a minute and consider the whole situation. Friends may be feeling that you're rushing into something or losing perspective.

It means to consider your surroundings literally but conversationally it would be the equivalent of "open your eyes" as in see what is occurring in front of you. Your friend could try to be warning you of something which you're unaware of just not in so many words.
