Foramina are openings in bones through which certain structures like nerves or blood vessels pass. C4-5 are the cervical vertebrae, the bones of the part of the spine that forms the neck. Patent, means that the foramina are not narrowed or blocked and nerves passing through them are not compressed.

The foramina refer to the small holes in the spine between the front and back parts of the discs in the vertebrae. Patent foramina mean that the bone that makes these holes are intact, not broken, not fractured, etc. and the blood vessels in these foramina are not damaged. This is good; if it were broken, the vessels could bleed.

If they collapse or become encrusted with calcium deposits it can cause pain and even possible paralysis. C4-5 is cervical, or neck, in the area of your shoulders.
