APA reports have a title page, abstract page (in which you summarize the main points of your research), the main body, and a reference page. All pages should have one inch margins on all sides, use a standard font (like Times New Roman) sized 10-12 pts, and include a header. There are lots of writing guides and samples online.

Yes there is a guideline online and there are also books available. APA requires references listed at the end of the report. APA also requires citations if needed.
