As with any legal document or citation, the best thing to do is to either contact the person who "awarded" it directly, or to contact an attorney. Typically laws and regulations vary by county and state. In this case I would personally contact your municipal court for more information here.

There are several things you can do. First call the employer and ask if they have been mailed out yet, if so make sure they have your correct address and ask them to send you a new copy. If this does not work, you may contact the IRS and request a transcript of your W2 records

Contact your employer and request your W2 form. They are required to give it to you. If that fails, use your last paystub from last year for the information.

The first thing to do is to make every effort to obtain an original copy of the W-2. Write a letter, make a phone call, and send an email, twice. Track it all. Then file a substitute W-2 with your pay stubs if ALL other options are exhausted.
