Righty tightly, lefty loosy. Use ribber gloves or rosin on your hands for traction, and make sure that your stud is clean. A dab of pain killing antibiotic on the peircing and studscrew can be very helpful If you have a good grasp rotate firmly to the left, and take care not to yank or irritate.

When the ball on my lip ring is too difficult to get off, I use a shirt or similar fabric to get a decent grip on it.

I've never had a lip stud but I have seen others with it. I'd definitely be afraid of swallowing the pieces should it come loose while I'm sleeping.
