Apply a thin layer of a skin bleaching cream least once a day. Keep hands out of the sun, and apply sunscreen when hands are exposed to sunlight.

Try using lemon juice to lighten the discoloration. Make sure you also use lotion though, lemon juice tends to dry out the skin.

Spots are caused by repeated sun damage, so for starters you need to begin using a good sunscreen on the back of your hands daily. Fade creams help a little.
