An infection can cause the swollen lips on a fish. It can be caused by a sharp substrate. The fish can accidentally cut its lips when it looks for food on the bottom. The bacteria of the water can infect in the fish that has a suppressed immune system. Stress in environment can cause this immune suppression.

This may just be irritation from digging around in the gravel. I know my convicts lips swell a bit at times from this. Was any of the substrate moved when you were changing the filter media? If so he may be rearranging it more to his liking. (which they do when they are spawning more than any other time)

I have never heard of swollen lips on a fish. I didnt even know fish had lips! If its mouth is swelled it could be from it banging its face on the glass.

bacterial infection can cause problems of that nature. you can find treatments in store such as pet land and pet smart. if you do not treat this the fish could die of starvation.
