Depending on the type of obstruction, there may be no symptoms at all, or intense pain. A blood test is needed to look for imbalances in creatinine and electrolytes. The intense pain would be located in the loin area and can radiate to the groin. The enlarged kidney may be sore upon examination.

Symptoms are less likely to show with unilateral hydronephrosis. If you're dealing with acute hydronephrosis, renal failure, enlarged kidneys, and pain in the lower back beneath the ribs are possible.
