You can apply for a visa online or through the Mexican Embassy/Consulate. Throughout 2010 the US State Department has issued a number of advisories and warnings re: travel in Mexico.

If your stay is 72 hours or less, you don't need a visa. If it's more then you'll need a tourist card, which you get when you cross the border.

There are no specific requirements for a visa, it is the same as getting one in the U.S. You must apply for a passport.
