No. The number one thing to remember is that if the puddle is truly water only, it's condensation from the a/c which is completely normal. The AC system is not filled with water so you know it can't leak. It must be cold in there!

No. Oftentimes, your A/C condenser will get very cold while its running and water will condense on the outside of it. Think about how water condenses on the outside of a glass with a cold drink in it on a summer day. Its normal.

If the puddle is under the car when it sits then it's just condensation from the A/C. If it's inside then there is a problem, probably a busted heater core.

All air conditioners will create a little bit of condensation, whether in a vehicle or in a house, however, it can cause major problems if it is overcondesating and producing enough water to damage your vehicle or cause rust. Have a certified professional check your AC unit if you have more than a small cup forming.
