Oh definitely be concerned if a four year old is not talking! By that age a toddler should constantly be talking and using sentences. You need to be in touch with your doctor who can refer you to a specialist for diagnosis if needed.

Yes. Children typically offer single words at about age one and sentences by about age two. If your child is not talking at all, there may be hearing or language processing problems. Your pediatrician should refer you to other specialists. If you have no pediatrician, contact a speech and language therapist.

You should have been concerned a few years ago! You need to make an appointment with your child's Pediatrician to see if something is wrong with him/her. Speech delay can indicate a larger problem such as Autism, Learning Delays or even mild forms of Mental Retardation.

It may be that he is just not interested in speaking but you should be concerned. Take him to your pediatrician who can do a preliminary hearing check and then advise you as to what other steps you should take.
