The only currently-extant place in the United States called "Smalltown" is an unincorporated community in Barnstable County, Massachusetts, on upper Cape Cod. "Smalltown" is also commonly used as a metaphor for any small town, as in Justin Moore's song "Smalltown USA."

"Smalltown" usually refers to a particular way of thinking and is used to suggest that people with that kind of thinking come from a small town, i.e. not a city and they are not exposed to modern, sophisticated lives but live in an old fashioned way with generally conservative (at best) values. It is usually a negative comment.

There's one in NJ, it's also called midgetville. Supposedly, there is a bunch of violent midgets who live in little houses who will attack you if you enter their streets or town and your not a midget. I've had friends who have actually gotten hurt by them, so it is true.
