Is it acceptable to use a form for an affidavit for a will?

Yes. Has the date and states an affidavit or prepare a "Holographic Will" which is completely written and signed in the handwriting of the testator stating intended distribution of estate.

Yes, they are many attorneys and "How TO" legal books that carry the forms and are acceptable by the Courts as valid wills so long as they are properly notarized and signed by the parties. An Affidavit or a "Hand Drafted Will" is also acceptable by the Court.

It is acceptable to use a form as long as it complies to the state statutes where you will be having the will enforced.

Using an affidavit as a will is legal in some states, but talk to an attorney for the laws of your state. Even an affidavit will require witnesses.

The laws for wills vary from state to state. However, in general, form affidavits are acceptable for wills if they are properly signed and notarized and would otherwise be valid (in other words, the affidavit has to follow the state's rules for wills in general).

Yes it is so long as communicates what it is (in as much detail as possible) that you want. for example make sure you make some one the executor of your estate and ensure that your various wishes about property and finial wishes are carried out.
