I have a QDRO that has been accepted. I'm not a lawyer. How do I file it with the courts?

Depending on your state of residence, you will need to file for an order/decree/judgment formally approving the QDRO. While consulting an attorney (legal aid or otherwise) is highly recommended, visiting your state court's home website and consulting its guidelines on obtaining a decree/uncontested judgment may be a first step in getting your QDRO ordered.

You must fill our a general form affidavit as well as the required form supporting your claim (this will be where you record specific QDRO information). These forms can be picked up and submitted at the court of your jurisdiction
