Take off the old cushion and remove the stuffing then wash the old covers. You just put new fabric over the old once it is clean.

You will need enough fabric to cover the foam. If you don't have enough original get all new fabric. Basically you then wrap the cushion like it was a x-mas present and sew it shut tight. Put the seam in the back so it doesn't show.

The simplest way to do it at home involves a bit of stitching and cutting. Start by turning the upholstery inside out and stretching the fabric over the cushion until it is the desired tightness. Then, pin it in place. Once it is in place, sew a new seam line, cut the excess, and turn it right-side out again.

You can go to your local sewing store and find a slipcover pattern, pick out some materiel and make a cover for the whole couch. If you are just wanting to cover the loose cushions you could try tracing around cushion and creating your own pattern.
