If there is a large amount of concrete, and it is not reinforced with steel bars, the best way would be to remove it using a concrete breaker. Once removed install formwork and pour new concrete in thin layers, vibrating and tamping to remove air voids as you lay.

First you need to remove all of the old concrete using a jack hammer. Then you will be able to pour new concrete to replace the old.

It's best to let professionals do it. Trying to do it by yourself will cause multiple damages to your sidewalk and you could violate county ordinance laws.
