You must decide how you want the last name to look and sound. Do you want the maiden name to be first followed by the married name or the other way around? Then you must decide if you want to seperate it with a hyphen, or space. Most common way is maiden name-married name.

The most common way women keep their maiden names, is to substitute it for their middle name. You could also hyphenate with your last name first.
