How do you address an envelope?

In the upper left had corner of the envelope, list the name and address of the sender. In the center of the envelope, list the name and address of the recipient.

Your name, your street address, and your city, state and zip go in the top left corner in three lines in that order. The recipient's name, street address, and city, state and zip go about the middle of the envelope and a little to the right.
