Tomato juice, vodka, horse radish, hot sauce, Worcestershire sause, salt, pepper, lemon juice. Combine all of those in a glass and add celery to garnish.
The official recipe, according to IBA (International Bartenders' Association) is 45 ml vodka, 90 ml tomato juice and 15 ml lemon juice. Or, 3 parts vodka, 6 parts tomato juice and 1 part lemon juice.
