You must be paid for all overtime you work; although what constitutes overtime can vary from state to state. For example, in CA. overtime is anything over 8 hours per shift. In other states it may be anything over 40 hours per week.

Salaried employees are exempt from overtime. Hourly employees must be paid overtime at time and a half, if not more.

If an employee has work restrictions for medical reasons he might not have to work overtime by the day or by the week. This needs to be put in writing by a qualified professional but there are some reasons that can be kept private due to future employment and things being of a personal nature.

Employees can be exempt from receiving overtime, but I don't feel thats very fair. I won't work extra hours without a bonus.
