Can an insurance company require that you take a blood test to determine if there is nicotine in your system?

Insurance companies are private businesses which have the ability to deny you insurance or grant you insurance based on your medical history and how you react to many questions. Denial of testing is just as good as saying, maybe, and they will decide whether that means you are a higher risk or not.

Non-smokers have less health problems and usually live longer so if you are applying for a type of insurance, the company could want to be sure you are truthful.
