The ever popular chick with the Lollipop in GTA IV Is called Lola Del Rio. She featured on concept art & box art for the retail game. She doesn't actually appear in the game, but a quick search on the police computers in the game confirms that she's hooker. The real life model is unknown.

I don't know if there is a real life model for the girl you are talking about. But, the name of the woman with the lollipop in the game is Candy Suxxx, if I'm not mistaken.
