You can down download the form from the California State Franchise Tax Board's website (ftb.ca.gov). There they have all of the forms that you will need to file your California State Return.
You can find them at the Post Office, Tax Preparation Offices (they have to provide at no cost) or you can go to the CA Franchise Tax Boards web site and download and print a copy.
