What is meant by a "hostile work environment?"

I would appreciate some examples.

If your boss threatens you with aggression or physical harm if you don't complete a task or do something at work.

If all your coworkers make remarks about your appearance, leave pictures of you on your desk that have been altered to make you look ugly, leave notes about how ugly you are, etc, that would be a good example of you having to work in a hostile work environment. Lawsuits can be brought over this.
