Physical testing for fire fighters usually involves a job simulation type test. It will usually be about 5 minutes duration and involve running, lifting, climbing etc while in bunker gear.
One grueling tasks fire fighters have to go through is extreme temperatures. Most physicals test for the bodies temperament to these extremes. Other tests such running two miles in heavy uniform are also used to test this.
First you have a health physical. Blood pressure, weight, body mass, heart, eyes, hearing. The you have an endurance test, lifting weight, climbing, and testing just how much you will be able to handle fire situations.
You must be able to show strength and confidence via ladder climbing, evacuating casualties, assembling and carrying equipment. All have weight added and may be done as a timed event.
