The initial outbreak of herpes (the most severe) will be represented by blisters in the genital regions. The blisters will turn into sores which will be surrounded by irritated skin.

An initial outbreak is typically the worst. You will see many discolored blister aroung your genitals that may or may not be sore and painful.

An initial outbreak of herpes produces a cold sore in the affected area, typically around the mouth, genitals, or anal area. It is possible to carry and spread herpes without obvious symptoms so it is best to get tested if you have reason to suspect herpes or other disease.
